Our Registered Nurse works closely with hospital staff to review health assessments, care requirements, and discharge readiness. A detailed discharge plan is created in consultation with key stakeholders to address all care needs.

Support Coordinator organises a case conference with families, allied health professionals, and hospital teams to collaboratively develop a comprehensive and tailored discharge plan.

A detailed support quote is prepared and submitted to the Rapid Hospital Discharge Program Coordinator, outlining the care model, accommodation needs, and associated costs.

Custom Support Model & Team Preparation
Our Clinical Lead Nurse carefully selects and trains a team of support workers to deliver the required high-intensity care. Each team member is equipped with the skills and knowledge necessary to meet the participant's specific needs.

Access to Accommodation Options
Supported Care provides access to a variety of NDIS-compliant accommodation options. Participants are allocated to the most suitable property based on their needs, preferences, and location, ensuring safety and comfort.

Discharge & Initiation of Supports
Upon discharged, participants begin receiving structured, high-intensity care delivered by our trained support workers under nurse supervision. Ongoing care is provided in medium-term accommodation, ensuring a stable and supportive environment.

Planning for Permanent Housing Solutions
We continue working with families, allied health, hospital teams and NDIS coordinators to develop a long-term housing plan, which may include Specialist Disability Accommodation (SDA) or other suitable arrangements tailored to the participant's goals and needs.
